It is celebrated worldwide, but as far as I can tell, the only place where it is celebrated on October 1st is Guatemala. World Children's day is a different date.

Care to elaborate? Ok, Guatemala, El Salvador, and Sri Lanka are the 3 listed on Wikipedia, so I guess you're right.
